This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.
Who this notice covers
This notice applies to the four Farmacia Carimas pharmacies in Puerto Rico — Toa Baja, Sabana Seca, Loíza, and Barranquitas — and to all health information the pharmacy keeps about you, on paper or electronic.
How we may use and disclose your health information
- For your treatment: filling your prescriptions, checking doses and interactions, and communicating with your doctor or other professionals who care for you.
- For payment: billing and collecting from your health plan or other payers for the medications and services you receive.
- For pharmacy operations: activities needed to run the pharmacy, such as quality control and compliance with the law.
- Reminders: we may call you to let you know your medication is ready or due for a refill.
- When the law requires it: we will disclose information when federal or Puerto Rico law requires it, including public health, safety, and court orders.
- Any other use or disclosure will be made only with your written authorization, which you may revoke at any time.
Your rights
- See and get a copy of your pharmacy record.
- Ask us to correct information that is incomplete or incorrect.
- Ask for a list of certain disclosures we have made of your information.
- Ask for limits on how we use or share your information.
- Ask us to contact you in a specific way or at a specific place.
- Receive a paper copy of this notice at any of the pharmacies, even if you read it here.
- File a complaint without it affecting your service.
Our duties
- The law requires us to keep your health information private, to give you this notice, and to follow the notice currently in effect.
- If a breach affects your information, we will notify you as the law requires.
- We may change this notice. The current version will be available at each pharmacy and on this page.
Complaints and questions
If you believe your privacy rights were violated, or you have questions about this notice, speak with the supervising pharmacist (farmacéutico regente) at your pharmacy.
You may also file a complaint with the federal Department of Health and Human Services, Office for Civil Rights (HHS): www.hhs.gov/ocr or 1-800-368-1019. Filing a complaint never affects your service at the pharmacy.
